SOC Narrative

Patient and (caregiver/spouse/relative/friend-write in names) available during interview. Patient verbally permitted (caregiver/relative/spouse/friend) to be present during initial assessment after HIPAA information in relation to confidentiality explained. Admission packet explained including Advance Directive & Patient Rights & Responsibilities. Copies of forms and admission documents given after explanation and patient’s signature acquired for the consent. Educational teachings given in relation to: Home safety, Infection Control and Fall Precautions. Explained plan of care and next visit schedule (and recommended discipline-add if any-delete this if not) and importance of PCP/Specialist’s follow-up appointments. SN instructed patient on importance of receiving influenza, pneumococcal and shingles vaccines, if able. Also instructed on after hours urgent and emergent needs and where to go. Instructed to always inform office if for any change in status or if hospitalization occurs. Medication reviewed and instructed on dosage, route, frequency & medication.
