Discharge Instructions

Patient aware of discharge from home health today. Provided the following discharge instructions: Personal Care / Activity: As tolerated. Notify the physician for any sudden weakness, loss of endurance, and unexplained fatigue or exhaustion. Follow the diet as prescribed by your physician and as instructed: Elimination: No restrictions. Notify physician for severe constipation, uncontrollable diarrhea, or any sudden and drastic change in elimination pattern. Call doctor’s office to make an appointment to be seen in:1-2 weeks. Call your physician if you have the following problems: shortness of breath, productive cough, lightheadedness, dizziness, nausea and vomiting. Instructed to continue current medication. Provided community resources. Instructed to call our office if further home health care services is needed in the future at (enter agency phone).
